Community Choices Waiver

Community Choices Waiver is a service that is provided by the state of Wyoming's Medicaid plan, allowing elderly or disabled individuals to continue living in a community setting such as their own home, the home of a family member or friend, or an assisted living facility. The program does require that financial and medical eligibilities be met.  B.A.C.A works with all clients and families through the eligibility determination processes. Upon an approved eligibility B.A.C.A. will then assist in developing and monitoring a person-centered plan of care that is tailored to an individual's needs and wishes.

Community Choices Waiver is a program that has allowed us to assist in fulfilling the wish that most of us have in common...to be able to live within our own homes. With Community Choices waiver we are able to provide assistance with personal emergency response systems, home delivered meals, skilled nursing, in home CNA assistance, assistance with personal supports, and homemaking. Through referrals with community partners, we can assist with getting adaptive equipment, housing assistance, transportation assistance, technology assistance, and more.

Why is Case Management important?

Case Management services are provided in the best interests of the client. Emphasizing confidentiality, respect of the client's rights and dignity, and enhancement of the clients humanity and self-esteem.

At B.A.C.A we maintain an ethical and professional relationship with our clients that includes an intake assessment of our clients' strengths, finances, adherence to medication, medical insurance status, nutrition, transportation, and basic needs. We then take this information and work with the client to assist them in maximizing their community as well as their strengths.

​

While all plans are person centered and require the client to identify what they need and how those needs can be met, B.A.C.A. will help to ensure that this goes as smooth as possible while providing advocacy, insight and guidance. We hope to see all of our clients being successful in the community while also being as independent as possible.

B.A.C.A Case Management is a small business that is owned and operated by Desiree Baca. Desiree has been involved in Case Management since 2014 and prior to that was in various parts of the medical field for 15 years. In 2015 Desiree graduated from the University of Wyoming, earning a Bachelors degree in Social Work with Honors. 

 

Desiree would like those she works with and beside to know her primary goal for B.A.C.A. is to provide a service that is rooted in integrity, empathy and compassion.
